These assessments and strategies create an important opportunity to improve the health of communities. They ensure that hospitals have the information they need to provide community benefits that meet the needs of their communities. They also provide an opportunity to improve coordination of hospital community benefits with other efforts to improve community health. By statute, the CHNAs must take into account input from “persons who represent the broad interests of the community served by the hospital facility, including those with special knowledge of or expertise in public health.”
In the course of receiving services from Montgomery General Health Care System, Inc., you will provide us with personal information about your health, with the understanding that this information will be kept confidential. We may also obtain information about your health from examinations, tests, or from others who have provided you with care. This notice of privacy practices is intended to inform you of the ways we may use your information and the occasions on which we may disclose this information to others.
We use patients’ information when providing treatment, and we disclose patients’ information to other health care providers to assist them in providing you with treatment.
